• Physical – provide paperwork to school nurse or bring to tryouts/first practice
• Recert Form – Download PDF (bring to tryouts/first practice)
• Concussion Form – Download PDF (bring to tryouts/first practice)
• Sports Fee: $50 – (bring to tryouts/first practice)
